Lockout/Tagout (LOTO) Training: Requirements, Refresher Cadence and How to Reach Every Shift

Lockout/tagout training teaches workers to isolate and control hazardous energy - electrical, mechanical, hydraulic, pneumatic, chemical, thermal, gravitational - before servicing or maintaining equipment, so that machines cannot start up or release stored energy while someone is inside them. In the US it is governed by OSHA 29 CFR 1910.147 (The Control of Hazardous Energy), and it has sat in OSHA's ten most-cited standards for over a decade, with training and periodic-inspection failures among the most common citation items. Every industrialised country has an equivalent, and every one of them turns on the same weak point: the training exists, but it doesn't reach the night shift, the contractor, the new operator, or the worker whose first language isn't the one the procedure is written in.

The three employee categories and what each must be trained on
OSHA's standard defines three groups, and the training obligation differs for each. Getting the categories right is half of compliance.
Authorized employees - those who lock out or tag out machines to perform servicing or maintenance. Training must cover:
- Recognition of applicable hazardous energy sources
- The type and magnitude of the energy in the workplace
- The methods and means for energy isolation and control
- The specific written procedures for each machine (the machine-specific LOTO procedure is itself a requirement under 1910.147(c)(4))
- Application, removal and transfer of locks and tags, group lockout, shift changeover, and verification of isolation (try-out)
Affected employees - those who operate or work near equipment that may be locked out. Training covers the purpose and use of the energy-control procedure and, critically, the prohibition on attempting to restart or re-energise locked-out equipment.
Other employees - anyone who works in an area where energy-control procedures may be used. Training covers the procedure and the same prohibition on restarting.
Where tagout (rather than lockout) is used, all employees must additionally be trained on the limitations of tags: a tag is a warning, not a physical restraint; it must never be bypassed or removed; it must be legible and understood by all employees.
The standard also requires the employer to certify that training has been completed - a record containing each employee's name and the training date. That certification is the first thing an OSHA compliance officer asks for after an amputation or a struck-by incident involving machinery.
When retraining is required
Unlike some standards, 1910.147 does not specify a fixed retraining interval. Retraining is required when:
- There is a change in job assignment
- There is a change in machines, equipment or processes that presents a new hazard
- There is a change in the energy-control procedures
- The employer's periodic inspection - required at least annually for each procedure (1910.147(c)(6)) - reveals deviations from or inadequacies in the employee's knowledge or use of the procedure
- The employer has reason to believe there are deviations or inadequacies
In practice, most well-run plants pair the annual periodic inspection with an annual refresher for authorized employees, and treat the inspection's findings as the trigger for targeted retraining. Retraining must re-establish proficiency and introduce new or revised control methods - and it must be certified like initial training.
Global equivalents
LOTO is a US term, but the requirement is universal.
- EU: the Work Equipment Directive 2009/104/EC and the Machinery Regulation (EU) 2023/1230 require energy isolation and safe maintenance; national law (e.g., UK PUWER 1998 Regulation 19 on isolation from energy sources, Germany's BetrSichV and DGUV rules) adds the training duty. The general safety-training duty under Directive 89/391 covers it.

- Canada: CSA Z460 (Control of Hazardous Energy) is the reference standard, adopted by reference in most provinces.
- Australia: WHS Regulations and the Code of Practice for Managing Risks of Plant; isolation procedures are a standard expectation.

- Mexico: NOM-004-STPS (machinery) and NOM-029 (electrical maintenance).
- Gulf and Southeast Asia: most national OSH frameworks reference OSHA or ISO 45001-aligned isolation practices; multinational buyers' audits enforce them.
The common thread: a documented, machine-specific procedure; trained and certified authorized personnel; awareness training for everyone else; and evidence.
Why LOTO training fails in practice
The gap between having a LOTO program and having a LOTO-competent workforce shows up in incident investigations again and again. Five patterns:
1. It's trained once, at induction, in a classroom. The new operator hears about LOTO in week one alongside forty other topics. Six months later they're asked to clear a jam on a conveyor at 3am.
2. It doesn't reach contractors. Maintenance contractors, cleaning crews and installation teams change weekly and are rarely in the plant's HR or LMS. The standard explicitly requires the host employer and contractor to inform each other of their respective procedures - and in practice this is a signature on a gate-pass form. 3. Language. A procedure written in English (or Portuguese, or German) taught to a workforce that speaks six languages produces "trained" workers who cannot explain the try-out step. 4. Machine-specific procedures aren't at the machine. The procedure lives in a binder in the maintenance office. The worker at the machine has the general training and no specific steps.
5. The periodic inspection is a paperwork event. A supervisor signs a form annually without observing an actual lockout. Deviations are never found, so retraining is never triggered - until an incident triggers it.

A 45-day LOTO training rollout for one plant
Days 1–10. Inventory: machines with written procedures (and those without), authorized-employee list by shift, affected/other populations including contractors, languages on site. Fix missing procedures first - training can't cover what isn't written.
